Is my image uploaded to a server?

No. All rotation and flipping is done entirely in your browser using the HTML5 Canvas API. Your images never leave your device.

Will rotating a PNG preserve the transparent background?

Yes. When you export as PNG, the canvas background remains transparent, so any transparent areas in the original image are preserved.

What is the difference between the preset buttons and the custom angle slider?

The preset buttons (90 left, 90 right, 180) apply an incremental rotation on top of the current state. The custom angle slider sets an absolute angle relative to the original image, which is useful for fine corrections.

Which output formats are supported?
