What format should the secret key be?

The key should be a Base32-encoded string (letters A-Z and numbers 2-7), like JBSWY3DPEHPK3PXP. Spaces are ignored automatically.

What algorithm does this use?

SHA1-HMAC with a 30-second period and 6-digit output, which is the standard TOTP (RFC 6238) used by most authenticator apps.
