How does this underline work without HTML?

Each character is followed by U+0332 (COMBINING LOW LINE), a zero-width Unicode diacritic that renders as an underline beneath the preceding character. Because it is a real Unicode code point, platforms that strip HTML and markdown still display the underline.

Will it display on all devices and apps?

The combining underline is supported by all modern Unicode-aware apps, browsers, and operating systems. A small number of very old or limited-font environments may not render it correctly and will display the base character without the underline.

Do spaces get underlined too?

Yes — by default every character including spaces receives the combining mark, which produces a continuous underline. If you prefer only letters and digits to be underlined, use the "Letters only" toggle.
