Fluid Deficit NPO Calculator

Estimate how much IV fluid a patient needs to replace after being NPO, using weight and fasting duration.

Patient Parameters

Enter patient parameters and click Calculate.

Clinical disclaimer: This tool provides an estimate only. Fluid management decisions must account for comorbidities, hemodynamic status, ongoing losses, and institutional protocols. Always apply clinical judgment and consult appropriate anesthesia personnel.

Summary

Estimate how much IV fluid a patient needs to replace after being NPO, using weight and fasting duration.

How it works

  1. Enter the patient's weight in kilograms.
  2. Enter the number of hours the patient has been NPO (fasting).
  3. Select the patient's baseline maintenance fluid rate method (Holiday-Segar or weight-based).
  4. The calculator multiplies the hourly maintenance rate by NPO hours to get total deficit.
  5. It then outputs the deficit and the standard 3-hour replacement schedule (50% in hour 1, 25% in hour 2, 25% in hour 3).

Use cases

  • Estimating pre-operative IV fluid deficit for elective surgical patients.
  • Planning intraoperative fluid replacement strategy with the anesthesia team.
  • Teaching anesthesia residents the Holliday-Segar maintenance formula.
  • Quick bedside calculation when electronic records are unavailable.
  • Assessing fluid needs for pediatric patients with extended fasting times.
  • Documenting fluid deficit rationale in pre-op anesthesia notes.
