What does FLIPI stand for?

FLIPI stands for Follicular Lymphoma International Prognostic Index. It was derived from a large international retrospective cohort study published in 2004 and is widely used in clinical practice to estimate overall survival in follicular lymphoma.

What are the FLIPI risk groups and survival estimates?

Low risk (0–1 points): approximately 91% 5-year OS and 71% 10-year OS. Intermediate risk (2 points): approximately 78% 5-year OS and 51% 10-year OS. High risk (3–5 points): approximately 53% 5-year OS and 36% 10-year OS. These figures are historical estimates from the pre-rituximab era; outcomes are better with modern chemoimmunotherapy.

How many nodal areas are counted for FLIPI?

The FLIPI study defined 17 nodal areas based on the Ann Arbor system, including cervical, axillary, mediastinal, hilar, para-aortic, mesenteric, iliac, and inguinal regions, as well as the spleen. More than 4 involved areas scores the adverse point.

Is FLIPI still used alongside FLIPI-2 and m7-FLIPI?

Yes. The original FLIPI remains the most widely cited tool and is incorporated into many clinical guidelines. FLIPI-2 uses different parameters (beta-2 microglobulin, longest node diameter, bone marrow involvement, hemoglobin, age) and is also validated. The m7-FLIPI adds molecular mutation data. Clinicians often select the index based on available data.
