How it works

  1. On an anterior-posterior spinal X-ray, identify the most tilted vertebra at the top of the curve (upper end vertebra) and the most tilted vertebra at the bottom (lower end vertebra).
  2. Measure the angle each end-plate makes with the horizontal. Record both values in degrees.
  3. Enter the upper end-plate angle and the lower end-plate angle into the calculator.
  4. The Cobb angle equals the sum of the two tilt values (they lean toward the apex of the curve, so they add).
  5. The result is classified from Normal variation through Very Severe to indicate observation, bracing, or surgical thresholds.
