What is the Centor Score?

The Centor Score is a clinical scoring system developed to predict Group A Streptococcal (GAS) pharyngitis. The original four criteria (exudate, lymphadenopathy, absence of cough, fever) were later modified by McIsaac to add patient age, improving accuracy across age groups.

How is the score interpreted?

Higher scores indicate a higher pre-test probability. Current testing decisions depend on age, symptoms, local guidance, and availability of rapid antigen or molecular testing; antibiotics are for confirmed GAS under the applicable guideline.

Does this tool replace a throat swab?

No. The Centor Score guides clinical decision-making but does not replace microbiological testing. Rapid antigen detection tests (RADT) and throat cultures remain the definitive methods for diagnosing strep pharyngitis.

Why does age affect the score?

The McIsaac modification adds +1 for age 3–14 years (higher strep prevalence in children) and −1 for age ≥ 45 years (lower prevalence in older adults). Ages 15–44 carry no age-related modifier.

Is this tool for adults only?

No. The modified McIsaac score applies to patients 3 years and older. It is not validated for children under 3, who rarely develop GAS pharyngitis.

Does a high score mean the patient definitely has strep?

Not definitively. Even at a score of 5, viral pharyngitis remains possible. The score provides probability estimates, not certainty. Clinical context, local epidemiology, and patient factors should always inform the final decision.
