Summary

Enter five clinical parameters to calculate the ABCD2 score and estimate 2-day stroke risk after a TIA.

How it works

  1. Select the patient's age: 60 or older scores 1 point, under 60 scores 0.
  2. Enter whether blood pressure at presentation was elevated (systolic ≥ 140 mmHg or diastolic ≥ 90 mmHg).
  3. Choose the dominant clinical feature: unilateral weakness scores 2, speech disturbance without weakness scores 1, other features score 0.
  4. Select symptom duration: 60 minutes or longer scores 2, 10–59 minutes scores 1, under 10 minutes scores 0.
  5. Indicate whether the patient has a history of diabetes.
  6. The calculator sums the points and displays the total score with corresponding 2-day stroke risk estimate.

Use cases

  • Triage TIA patients in the emergency department for urgent vs. routine workup.
  • Support clinical decision-making for hospital admission after TIA.
  • Communicate quantified stroke risk to patients and families.
  • Identify high-risk patients who need immediate neurology consultation.
  • Document risk stratification in clinical notes.
  • Teach residents and students about TIA risk factors.
